This is all I could find about Living Wills on the Elder Affairs

Overall I’d have to say the State of Florida has not done a very good job providing information and services concerning Living Wills that is easy to locate and utilize. Considering the recent Terri Schiavo case, what they offer seems fairly inadequate to me. Why not have one easy to locate webpage that tells you what a Living Will is, what the law says about it and an online form to fill out and print? If the government all of a sudden thinks Living Wills are of such importance, why not demonstrate that belief by providing an easy to locate and all-inclusive webpage about Living Wills and link to it on the State of Florida’s “official” website?

Elder Affairs
Living Wills- A document that formalizes an individual’s wishes regarding the medical care that is to be used or withheld if he or she becomes incapacitated or unable to make his/her own decisions. Many living wills include “do not resuscitate” orders that spell out under what circumstances an individual does not want to be revived.
